I serve as the primary point of contact for all medicine-related queries, offering expert advice and guidance to patients and healthcare teams. Additionally, I actively contribute to medicines management and audits, optimising the safe and cost-effective use of medications, thus improving the overall quality of patient care.
I handle medication reconciliation for patients recently discharged from the hospital, ensuring a smooth continuum of care and minimising medication-related risks.
Collaborating closely with practice nurses, I manage and prescribe for long-term conditions, ensuring comprehensive patient care. I proactively audit patients that are high risk of cardiovascular disease and offer medications and lifestyle advice to reduce disease risks.
